Whether you are inquiring about our school or enrolling a new student, we are happy to provide you with the following information. We are extremely proud of our school, our staff, and our community.   • Complete the SJA online registration form
  • New families: First, must make an account through FACTS. Second, create a payment plan for their tuition and book fees. After that, the Business Office will fill in the proper costs and finalize it.

    Current families who already have an account in FACTS need only to check their Personal information for accuracy, add an additional child if applicable, and create a payment plan for tuition and books. After that, the Business Office will fill in the proper costs and finalize it.

    All families pay Registration, Lunch Fees, field trips, and Child Care through incidental payments which are invoiced from the Business Office through FACTS.

  • Submit a copy of original birth certificate
  • Submit a copy of student's social security card
  • Submit an updated immunization certificate
  • Submit a school physical form
  • Submit a Kentucky eye exam form (Kindergarten only)
  • Submit a copy of baptismal certificate (Catholic students only)
  • Transfer Students Need:

    In addition to the above, transfer of records form signed by the parent 


    Note: Students entering kindergarten must be five (5) years of age no later than August 1st. All forms and payment should be submitted to the main office. All potential students should be familiar with the school dress code and have reviewed the SJA Handbook.
